    Biomarkers in bladder pain syndrome: A new narrative

    Bladder pain syndrome, or interstitial cystitis (IC/BPS), is chronic pain disorder of unknown etiology that negatively affects quality of life. Its definition has changed several times in the past 40years and is not homogeneous throughout guidelines. Proposed pathophysiologic mechanisms for IC/BPS include central neurologic changes, inflammation, urothelial dysfunction, autoimmunity, and infection, among others. In order to aid in disease diagnosis and follow-up, researchers have engaged in discovering biomarkers for this disease. Potential biomarkers for IC/BPS found in the bladder wall, urine, serum, and stool have been reported, but none has proven to be clinically useful so far. In this chapter, biomarkers are explored in relation to possible clinical applications and role in disease pathomechanisms. In order for reports to be comparable and for us to progress in determining useful biomarkers for this disease, the definition of IC/BPS should be standardized. © 2022 Elsevier Inc.     The human vaginal microbiome

    Women have developed a unique vaginal microbiome, distinct from all other mammals, to prevent infection and to protect both the mother and fetus during pregnancy. Lactobacilli, predominantly L. crispatus and L. iners, are numerically dominant in the vagina, and their presence increases further during pregnancy. Their predominance may prevent other microorganisms, from adhering to the vaginal epithelium and initiating an infection. They produce lactic acid that acidifies the vagina and inhibits the growth of other microorganisms. Lactic acid also specifically inactivates a number of sexually transmitted viral and bacterial pathogens, such as human immunodeficiency virus, herpesvirus, Chlamydia trachomatis, Neisseria gonorrhoeae, and the bacteria associated with bacterial vaginosis. Lactic acid also possesses immunological properties that prevent activation of proinflammatory immunity in the vagina, while concomitantly promoting the release of antibacterial products from vaginal epithelial cells. L. crispatus also uniquely promotes vaginal health, by inducing autophagy in epithelial cells. © 2019 Elsevier Inc. All rights reserved.

    Surgical pinealectomy in rats

    To report a new, direct visual approach for rat pinealectomy. The rat was weighted and anesthetized intraperitoneally with 15 mg/kg xylazine and 30 mg/kg ketamine. The animal was fastened to a dissection table, an incision was made in the skin and the subcutaneous tissue, bringing the lambda into view, under a stereomicroscope with magnification 10 - 40x. The skullcap was opened with a dental drill, bringing the cerebral hemispheres and the superior sagittal sinus into view. The pineal gland, located under the venous sinus, was removed in a single piece using tweezers. Next, the bone fragment was returned to its place and the surgical layers were sutured. This new technique is easy to be done, avoids bleedings and removes only the pineal gland without damage to the remaining encephalon. In addition it makes possible the achievement of a sham surgery, allowing the pineal gland to remain intact. The proposed technique intends to facilitate studies aiming to better understanding the complexity and importance of the pineal gland on reproductive and other body systems. © 2013 Nova Science Publishers, Inc.     Several immunological abnormalities were described in endometriosis, but their role in the pathogenesis of the disease is not completely understood. Although peritoneal immunosurveillance is mostly defective, some aspects of the immune system are upregulated, such as the widespread polyclonal activation of B lymphocytes with antibody production. Antiendometrial antibodies, IgA and IgG have been shown to be increased in endometriosis, and also anti-nuclear, antiphospholipid, and anti-DNA. It has been proposed that endometriosis may have an autoimmune etiology, as it shares common aspects with autoimmune diseases. Most drugs effective at treating endometriosis are hormonal and contraceptive, so targeting immune cells could be an alternative strategy. Btk inhibitor Ibrutinib inactivates B lymphocytes and has been recently shown to be effective in controlling disease progression in mice. Further studies should evaluate B cells subtypes and drugs that target these cells to better understand the pathogenesis and to develop new approaches to treat the disease. © 2022 Elsevier Inc.
