    Hypertension in the Developing World

    Hypertension is a major global chronic noncommunicable disease (NCD). Despite continued efforts to establish public policies to ensure surveillance and monitoring, the absolute numbers of patients affected by hypertension in low- and middle-income countries are likely to grow, as economic improvement favors longer life expectancy. Increasing longevity provides longer periods of exposure to the risk factors of cardiovascular disease (CVD), resulting in a greater probability of clinical manifestation of CVD events, especially because of the lack of awareness and insufficient treatment of the affected individuals. The survivors of an economic transition period are more likely to present the phenotype of lower birth weight coupled with either stunting or a higher body mass index in childhood or adulthood, which appears to be associated with the highest risks of morbid cardiovascular, renal, and metabolic outcomes into adulthood. The combination of population-wide and individual interventions may save millions of lives and considerably reduce human suffering from NCDs. © Springer Nature Switzerland AG 2023.

    Dermatological Manifestations in Fabry Disease

    Dermatological manifestations are the most initial alterations to be noticed in patients with Fabry disease (FD), and their recognition is necessary for an early diagnosis. They are angiokeratomas, sweating alterations, acroparesthesias, lymphedema, and facial dysmorphia.     Transcriptomics of Neonatal and Infant Human Thymus

    Genomic studies on human thymic development, growth, and involution are crucially demanded for understanding the processes of immunosenescence and inflammaging. Age-related thymus decline is mainly associated with adaptative immune system malfunction, leading to autoimmunity and ineffective responses to infections. Thymic explants obtained at cardiac surgery constitute an asset for transcriptomic studies of human thymus at critical stages of organ growth and initial decline. This chapter deals with the interpretation of whole thymic tissue transcriptomic datasets using modular transcriptional analysis techniques and integrative mRNA–miRNA–transcription factor (TF) co-expression analysis. © The Editor(s) (if applicable) and The Author(s), under exclusive license to Springer Nature Switzerland AG 2014, 2022.

    Methods for Gene Co-expression Network Visualization and Analysis

    Gene network analysis is an important tool for studying the changes in steady states that characterize cell functional properties, the genome-environment interplay, and the health-disease transitions. Moreover, gene co-expression and protein–protein interaction (PPI) data can be integrated with clinical, histopathological, and imaging information – a current practice in systems biology – leading, for instance, to the identification of unique and common drivers for disease conditions. In this chapter the fundamentals for gene co-expression network construction, visualization, and analysis are revised, emphasizing its scale-free nature, the measures that express its most relevant topological features, and methods for network validation. © The Editor(s) (if applicable) and The Author(s), under exclusive license to Springer Nature Switzerland AG 2014, 2022.

    Lupus Nephritis

    Childhood-onset systemic lupus erythematosus (cSLE) is a subset of SLE with onset prior to 16-18 years of age. Around 15% of patients with SLE present in childhood; patients with cSLE have a greater genetic component to their disease, more systemic involvement, and more severe disease course including higher risk for developing nephritis. Patients with cSLE develop loss of tolerance to self at an earlier time than patients with adult-onset SLE, or loss of tolerance progresses sooner to autoantibody production, immune complex deposition, and immune-mediated tissue injury. We review the diagnosis, classification criteria, and management of kidney disease in cSLE, by pediatric nephrologists and rheumatologists. Although the evidence basis for management often primarily relies on adult studies, there is growing literature on cSLE around the world. Fortunately, 5-and 10-year mortality is lower than in adult-onset SLE, and patient and renal survival has improved over the years. However, relapses are common, especially during the transition from adolescence to adulthood. Patients with cSLE also accrue organ damage and co-morbid conditions from lupus and their therapies, including cardiovascular, infectious, ocular, skeletal, growth, endocrine, and reproductive complications. © Springer Nature Switzerland AG 2009, 2016, 2022.
